Tri-City Hospice

Hospice is compassionate and professional health care available for anyone regardless of diagnosis. Its goal is to enhance quality of life through management of disease symptoms and relief of the fear and uncertainty associated with the dying process.

Help in Managing Care

The last months, weeks and days of a person's life are precious. For the terminally ill, this time should be a celebration of life and a loving time shared with friends and family. To ensure this experience, Tri-City Hospice provides specialized medical and social services, home health aides, counseling and volunteers to the person at home.

Tri-City Hospice is committed to:

  • Controlling the symptoms of the dying patient

  • Easing emotional issues for the patient and family

At Home or at Another Facility

Tri-City Hospice provides services to patients at their residence or at the skilled nursing facility or other setting where they live. By providing care in the patient's place of residence, the Hospice helps the terminally ill patient maintain dignity and live each day as fully as possible.

Expert Care by a Professional Team

Tri-City Hospice care is provided by a team of health care professionals specifically trained in comfort care. Each team member visits the patient at home during scheduled times to provide support, teaching and services. A team may include:

  • Physician

  • Registered Nurse

  • Clinical Social Worker

  • Home Health Aide

  • Chaplain

  • Bereavement Counselor

  • Other Volunteers

Tri-City Hospice provides quality health care services in North County without discrimination on the basis of race, color, religion, national origin, sex, age or financial position. Tri-City Hospice provides bereavement services for patients in the acute facility through our "Lumina Program", which supports patients at end of life who have not elected hospice services. In addition, our children's bereavement program, "Heart-to-Heart" provides support for school age children, impacted by the death of a loved one, who can be overlooked during the grieving process. The Heart-to-Heart services are provided in a collaborative relationship with local school counselors.
